This is picture of George Fay with an old woman, thought to be his maternal grandmother (i.e. Sarah A. Hand's mother), since his father's mother (Mary McGinn) is believed to have died in 1915. This picture was taken in Ireland, about 1933. George had ideas of fighting against Fascism in Spain, but any enthusiasm wained, and he went to Ireland instead. The old lady appears to be at least in her late eighties, which would be correct for her likely birthdate (1845-1850). (The identities of the woman and man are unknown.)
